Health Clinic - Special Health Care Needs

If your child will be returning to school with a cast, crutches, new medication or newly diagnosed illness, or requiring a medical procedure such as nebulizer treatment, please call the clinic as soon as possible to discuss accommodations with the school nurse. It is important that the nurse is aware of students who are on crutches or who have a cast to ensure student safety.

SPORTS PHYSICALS

Middle School requires sports physicals every year. This will allow students to participate in the required after school sports.

Florida High School Athletic Association
FHSAA Preparticipation Physical Evaluation form must be returned to the Athletics Department or the Middle School Office.
